Unique Disability ID Card for Persons with Disabilities

What is UDID Card?

The UDID Card, or Unique Disability ID Card, is an initiative by the Department of Empowerment of Persons with Disabilities (DEPwD) under the Ministry of Social Justice and Empowerment, Government of India. This card aims to offer a standardized identity for individuals with disabilities, simplifying access to benefits and services. It features a unique identification number, personal information, and disability-specific details.

The Government of India maintains a database of all UDID Card holders, which aids in the development and implementation of future schemes. Upon applying for a UDID, individuals with disabilities receive a UDID ID Card. This card can be used to access various government benefits.

Eligibility & Qualifying Disabilities

Only individuals with specific types of disabilities outlined in the Persons with Disabilities Act, 1995 are eligible:

👁️ Blindness
Total blindness or visual acuity ≤6/60
👓 Low Vision
Visual acuity <6/18 but >3/60
🧠 Intellectual Disability
Limitations in intellectual functioning
🦽 Cerebral Palsy
Motor impairments due to brain damage
🦻 Hearing Impairment
Significant loss of hearing ability
🧠 Mental Illness
Psychological disorders affecting daily functioning
🦵 Locomotor Disability
Affects movement or limb control
🩺 Leprosy (Cured)
Residual effects after treatment

Types of UDID Cards

🔵 Blue Card
Disability ≥80%
🟡 Yellow Card
Disability 40% - 80%
⚪ White Card

How to Apply

Step 1: Visit https://www.swavlambancard.gov.in/

Step 2: Click on "Apply for UDID" on the homepage

Step 3: Registration Page opens - provide personal, disability, employment details

Step 4: Select hospital for assessment in your district

Step 5: Declare and submit

Step 6: Download Application receipt & take printout to hospital for verification

⏰ Note: Minimum verification time is typically 7-15 days depending on local medical facility.

Track Your Application

Visit the portal and click "Track your Application". Track using:

  • Enrollment Number
  • Mobile Number
  • Aadhar Number
  • UDID Number

Note: As per RPwD Rules 2017, disability certificate/UDID card can be issued by notified hospital at home district as well as the hospital where the PwD is taking medical treatment in connection with his/her disability.
