Requirements for Obtaining a UDID Card
Introduction
The UDID Card, or Unique Disability ID Card, is an initiative by the Department of Empowerment of Persons with Disabilities (DEPwD) under the Ministry of Social Justice and Empowerment, Government of India. This card aims to offer a standardized identity for individuals with disabilities, simplifying access to benefits and services. It features a unique identification number, personal information, and disability-specific details.
The Government of India maintains a database of all UDID Card holders, which aids in the development and implementation of future schemes. Upon applying for a UDID, individuals with disabilities receive a UDID ID Card. This card can be used to access various government benefits.
Goals of the UDID Card
- Establishing a national registry for Individuals with Disabilities and issuing a unique disability identification card to each person.
- To enhance transparency, efficiency, and ease in the distribution of various government benefits to Individuals with Disabilities.
- The card will also simplify the monitoring of beneficiaries’ financial and physical development.
- The database will support the government in crafting and carrying out various programs for Individuals with Disabilities.
Requirements for Obtaining a UDID Card
Some of qualifying disabilities is provided below:
Blindness
The term ‘blindness’ is defined as a condition where a person exhibits total blindness, visual acuity of 6/60 or 20/200 (Snellen) or worse in the better eye with corrective lenses and a visual field limitation of 20 degrees or less.
Low Vision
Low vision is defined as a condition where a person has visual acuity of less than 6/18 but better than 3/60 (20/70 but better than 20/400) in the better eye with corrective lenses or a significant reduction in the visual field that impacts daily activities, even with the use of corrective lenses or devices.
Mental Retardation
Mental Retardation (now commonly referred to as intellectual disability) is defined as a condition characterized by significant limitations in both intellectual functioning and adaptive behavior, which manifest before the age of 18. This includes difficulties in reasoning, problem-solving, and learning, along with challenges in daily living skills such as communication, self-care, and social interactions.
Cerebral Palsy
Cerebral Palsy is a neurological condition characterized by motor impairments due to brain damage occurring before or during birth. This condition affects movement, muscle tone, and coordination.
Hearing Impairment
Hearing Impairment refers to a significant loss of hearing ability that impacts communication, which can range from partial to total loss of hearing in one or both ears.
Mental Illness
Mental Illness refers to a range of psychological disorders that significantly impact a person’s thinking, mood, or behavior, and which interfere with their daily functioning and well-being.
Locomotor Disability
Locomotor Disability refers to a condition that affects a person’s ability to move or control their limbs, which may result in difficulties with walking, standing, or performing other physical tasks.
Leprosy (Being Cured)
Leprosy (being cured) refers to individuals who have successfully undergone treatment for leprosy (Hansen’s disease) and are no longer infectious, but may still experience residual effects or disabilities resulting from the disease.
Required Documents to Apply
- Scanned copy of recent photo
- Scanned copy of ID proof (Aadhar Card, Passport, Driver’s License, Voter ID Card, PAN Card, Birth Certificate or Disability Certificate issued by a competent authority(any one document))
- Scanned copy of Address proof (Aadhar Card, Utility Bills (such as electricity, water, or gas bills),Passport, Bank Statement or Passbook,Voter ID Card, Rent Agreement or Lease Agreement,Driving License,Certificate from a recognized authority or local government office(any one document))
- Scanned copy of Disability Certificate issued by a recognized medical authority or government body
- Scanned Image of Signature

How to Apply:
- Visit the link : https://www.swavlambancard.gov.in/
- On the Homepage click on “Apply for UDID”.
- Registration Page opens
- Provide your personal, disability, employment, and identification details
- Also need to enter the Hospital for assessment/issue of UDID card /disability certificate section by selecting the hospital in your District (Need to visit the hospital Selected for the verification process).
- Click the checkbox to declare that “what is stated above is true to the best of my knowledge” and submit.
- You will have an option of downloading the Application receipt.
- You will be receiving an application reference number(reference number can be received in registered number through phone number/ registered Email, Application receipt, UDID portal or by Contacting the Issuing Authority or or medical center where you applied for the UDID Card for assistance).
- Need to take the printout of Application Receipt to the Hospital for verification with Documents submitted.
Note: The minimum time for verification of the UDID (Unique Disability ID) Card by the doctor is typically around 7 to 15 days. However, this can vary based on the efficiency of the local medical facility or authority handling the process.
Tracking the Application:

- Click on Track your Application on the Home Page.
- Track your application through Enrollment Number, Mobile Number, Aadhar Number or UDID Number.
Download the e-UDID card:
- Visit the link: UDID (swavlambancard.gov.in)
- Enter the required details like enrollment number/UDID number, Date of birth
- Navigate to the section where you can check the status of your application. You should see an option to download the e-UDID once it is approved.
- Follow the prompts to download the e-UDID. You may be able to download it as a PDF file
- Once downloaded, you can print or save the e-UDID for your records.
Type of UDID Cards
- Blue Card: This card is issued if the person’s disability is 80% and above
- Yellow Card: This card is issued to the person whose disability is between 40% and 80%
- White Card: This card is issued to the person whose disability is below 40%
